What are the most important items to have before going into the underdark?

I would have to go with the Shield of Balduran as well and if you are evil and planning on killing any dragons down there, take all your dragon killing items from Windspar hills.

The Shield of Balduran for Beholders; the Amulet of Power, Shield of Harmony and Ring of Free Action for Mindflayers.

Shield of Balduran for Beholders is a MUST.

Potions of Genius to boost Intelligence and give you some breathing room against those Intelligence sucking attacks of the Mind flayers are a VERY overlooked and under-utilized item.

Excellent point. Those same potions will also permit spellcasters with lower intelligence to learn higher level spells.
