Playing on insane with Tactics mod ?
You get hit twice and it's 80 or more damage and you have 120 HP. Rangers have ironskins, mages stoneskins, but plain fighters ? OK, I know about Barbarians 20%, how about Berserker ? Or Kensai ? Sure you have a Yellow Dragon Armor and that helm and DOE but that is for one fighter. What if you have two ? Any ideas, tactics, suggestions ? Hardiness - well for two hits it works.

All I can tell you is just to get some different types of armor, gauntlets, boots, and so forth to protect against elements and physical damage.

The obvious answer would be "Don't get hit."
Invisibility Potions. Ranged attacks against melee opponents. Ilbratha. Fast Regeneration during the fight. Run away! Run away! Hit fast, hit hard, and hope you survive.

It is feasible to equip a Warrior to tank for you even on Insane Difficulty with the Tactics Mod installed. I dunno where you got those 80 damage hits from... but last I recalled, Admantium Golems do not roam the grounds of Waukeen Promenade.
Excluding Boss-Type Monsters, AC will save your tank, requiring monsters to score a critical hit in order to hit you. Note... this is against non-boss-type monsters. You will need to tool up differently in different situations. Resistances is also king. Do not worry about Magic Resistance, just slap on the Cloak of Mirroring. Concentrate more on elemental resistances, especially fire and acid. Ring of Gaxx will render you immuned to Poison. Cold damage is rare, excluding the Improved Ilynich Mod, and most electrical damage is minor. You will still need protection from psionics and mental spells, so this is where Chaotic Commands from your party Cleric comes in. Lilarcor, Equaliser and the Shield of Harmony can serve as substitutes. Ring of Free Action is essential. To protect against level-drain, we have the Mace of Disruption +2. It will not be as easy as the backline people, but that is the price you pay for being in the frontlines. Just tool up properly, and your tank can be near-nigh invulnerable as well. Hmm I encountered the same problem when I tried to create a new Monk in ToB on Insane with Tactics MOD. She started out with the boots of speed and run like an improved hasted, improved hasted character with boots of speed. Since I start directly itno ToB, I dont have any NPCs by my side and the only equipment i got is what the computer gave me. I stand no chance in the first encounter though. Had to give that one up [img]smile.gif[/img] I could of course just adjust the difficulty setting, but nope! Insane or nothing! I dont even think its doable.
