This seems like a very odd bug to me:
When I backstab a mage to death with my fighter/thief (dual, female, human, neutral good) his contingency casts mislead even as he is dying. This results in the pieces of the smashed wizard flying around and casting spells at me. These flesh-crumps block the space above the dead mage's gear and can neither be dispelled with dispel magic or breach-like spells nor with true sight or similar. The cannot be attacked physically or wounded with area effect spells (cloudkill, fireball, ...) I noticed this odd bug the first time when facing Pert the Adept. Thus I didn't get the Book of Infinite spells and had to go to Spellhold by convincing Desharik. When I returned (after Spellhold labyrinth and defeating Irenicus!!!) the invincible and invisible undead Perth was still there and would be casting confusion or charm spells on my party. However this issue does not seem to be related to Perth as I encountered it again with the Drow mage in the underdark prison. [ 02-19-2002: Message edited by: Faceman ]

True, it happened to my assasin/mage (lvl 16/20) a few times too.
The mage in the underdark pulled it on me, you kill the bastard. But his means he's below X% HP, so his chain-contingeny fires and there he is again .... However, i was able to kill his mislead, and then he became visible too. ( i cant remember if i casted true sight) This did seem to be a lesser version of him, with fewer spells, also, it only gave me 2000 XP, when i killed his lesser version. (im thinking a mislead / Simulacrum Combo, giving him an "extra" life) Sorry, i know im not helping by posting this, merely backing up your story

I couldn't get it working with my actual party.
My only mage is Imoen though. The problem with me is, that I can't dispel his Invisibility (Purge, True Sight, Detect Illusions nothing works) This brings me to another situation. Sometimes when I'm playing catch and hide with my thief to backstab all thos evil bastards they disappear. They don't attack and cannot be made visible, but they are still around, as I can't hide in Shadows or sav anymore. They can however be taken down easily with fireballs and cloudkills (if you know where they are)
