Loot is entirely random, although it's based on character level and other things (for example, Ratlings have a better chance of getting higher-level loot due to their "gold digger" intrinsic.

I think it's also dependent upon location, as the chests around Valeia probably will not have Tomokami Katanas and Robinswords in them.
