Slight correction.
When you talk to the genie in Irenicus' Dungeon you don't know Imoen is you're half sister. I believe the genie is implying that another human is in the other cell. Anyway, if I were actually there I would push the button so that the other person didn't have to suffer. Of course if I were a evil SOB then I wouldn't push the button and take my chances that the other cells inhabitant is a real nice person.

I actually think that it's implied that Imoen is the other cell.
Genetics aren't a factor here; Imoen is as much a sister to the PC than anyone else might be. And the exercise doesn't work if it's just some other human in the cell, that takes the 'fun' out of second-guessing and the remorse out of not pressing the button.
