From the choises I assume you are talking about Black Isle/Infinity engine games

I always start at normal - when I have beaten the game any subsequent games are played on insane

Normal if it's anything but new fighting game. If normal is to hard then i'll start at easy and work my way up.

No, games in general. I usually start at medium. Playing your first time through at easy really ruins the game.

I normally start a game at either Medium or Hard. If you start on easy it ruins the game like Dreamer128 said and if you start on Impossible it ruins the game also, coz you get too frustrated!

I usually start on one of the easier settings... it saves me from the absolute horror of looking for walkthroughs/cheats...

Of course it depends on why you play the game. If you play it for the challenge and the thrill of battle, medium is the best way to start. If you are looking for a good time, and a nice story-line. Easy is a more obvious choice. Personally, I enjoy fighting enemy's with a good A.I. Especially in FPS's.

I always start at medium on a game, mainly because i know that I am good at games and can get the hang of them quickly.

If its too hard/easy i change it.

I start on medium too. if I have a demo i might do easy just to get hang of controls if they're radically different from what I'm used to
