Has anyone ever had her randomly leave the Inn?

I got the message from the little girl that she was looking for me, so I went and talked with her and she told me about buying the bandit scalps. I went off to the Bandit camp, then went directly to Cloakwood, when I came back to town I went to the Inn to sell my scapls and she was gone. I only talked to her that once, so I don't think it was the "talk to her without a scalp and she leaves" thing...
Has anyone had this happen? Does she just leave on her own after you finish the next chapter? Or is it just some bug?

I'm playing BGT so it might just be a bug with that (I've run into a few others already).

She leaves after you've cleared the Bandit Camp (annoying, since that's precisely when you've got the greatest number of Scalps to sell). You can keep her there a bit longer, though, by not talking to her at ALL until after you've been to the Bandit Camp. You'll have to sell all your Scalps at the same time, but at least you sell them all.

Feldepost buys them too, as I recall, but he pays less for them.

What disturbs ME is the fact that these people can somehow tell a Bandit Scalp from the scalp of any other person. They could be encouraging people to run around scalping perfectly honest citizens in the middle of the night.

Whatcha have to do is clear out the camp EXCEPT for the Boss'es building. Then go to Beregost, sell the scalps, and return to fight the Boss.

I've cleared the whole bandit camp (inside and outside), am in Cloakwood now and she's still there

I know, I’m a little late here, but still…

Officer Vai leaves only after you clear the Cloakwood Mines, and even then, you get the last chance to talk with her (she tells you that she got orders to return to Baldur’s Gate and promises that you will look good in her report [+1 to reputation]). However, she won’t give you a last opportunity to sell more bandit scalps before she leaves, but then again, you shouldn’t have any scalps by that time (unless you met more bandits in random encounters/respawn areas).

If Officer Vai disappeared before you get a chance to talk with her after clearing Cloakwood Mines, then it is most likely a bug.

There is this one bandit that keeps respawning on the First Farm map. Every time I walk by that place, he's there again.

She wasn't there after I got back from Cloakwood. So not sure what that's about. But it doesn't matter any more, I've already sold the scalps to some merchant.

Perhaps you simply couldn’t find her? I had troubles with finding her once too, it’s rather dark not to mention crowded in the Jovial Jester, and Vai’s constant moving around doesn’t help either. Eventually, though, a persistent search should pay out, for Officer Vai supposed to be there until she tells you that she’s leaving for Baldur’s Gate.
