When I came to Athkatla first thing I did was go to government district to get Jan (and Viconia). Now I finished:
_Slums district, CC and its sewers. _Korgan's quest. _Jaheira's Harper quests. _Renal's Maevar's guild quest. _Waukeen's promenade and Circus. _Edwin's Nether scroll (currently he's now a woman). _Planar Sphere. _D'Arenise Keep. And other minor quests. Problem is Jan's sick child quest hasn't started. I've never had Jan before so never did the quest. Shouldn’t it have started by now? Or is it allignement/reputation dependent (I'm playing evil so keep rep around 5)?

There is no "required" trigger AFAIK. It simply happens some time in the game. Maybe it depends on how much time you actually spent IN Athkathla, maybe on Game Days maybe on real time.
I remember the Jansen quest starting mostly after returning from the "dark place". Sometimes it started earlier. Just wait for it to happen and if you want do some more of the Chapter2 quests. Or open up shadowkeeper, search for the variable and the set it via CLUAConsole.

It's *game time* base quest -- Jan needs to be in your party for about 15 or so game days (you cannot dump him in these 15 days, or else the time will be reset).
