Is that how I had a cloak of mirroring on the floor? I thought that I had dropped it from my inventory since I never used it, so I just let it sit there. I suppose I didn't do that, then. I never realized that this was a bug.

not dropped from your inventory; in the beginning of the battle, each of the five fragments take one thing from (if you have them of course): the Cloak, Crom Faeyr, Amulet of Power, Shield of Balduran, and Sanchuudoku. When you kill the one that steals the item, that item will be back to your inventory, except for the Cloak; it'll drop on the ground (on Irenicus' body).

I never go to buy the shield, and this time around I didn't go getting the Crom Faeyr forged. Actually, the only item I use is the Amulet.
