I found it a couple of years ago packaged with Soulbringer (which was hard to get in to but I finished it with much satisfaction.) I haven't seen it in standalone form though. The two cost me around $10 Canadian. I did not complete Planscape the first time I tried it, but I gave it a second try and liked it immensely.

I found a standalone in the bargainbins in some store i can't even remember. It's been more than two years ago now, and i have to say i haven't seen it in stores since, i guess i just got lucky that day. Also a reward for always checking the bargainbins whenever i run into em, 9 out of 10 times i find nothing noteworthy, but that 10th time something always pops up.

torment fixpack and official 1.1 patch:

there's other downloads there, too. platter and the crew keep it updated, so if you run into problems, they might have a fix for it.

The official patch is a must to get rid of the slowdowns in some areas of the game.
