Pagan just means you're not the dominant religion in the region (typically used in reference to christianity)

I've also heard it means you're not a member of one of the Big Three:
Christianity, Judiasm, or Islam.

In its original Latin meaning, it meant "Country dweller"... So during widespread conversion in cities, it's easy to see how the meaning of the word could be perverted. Easy to convert in cities, but out in the country they might either be difficult to reach or difficult to convince, ie "We must convert the pagans."
