I have a few questions about them
First, what level should I dual class my berserker to druid (I thought level 12, but I'm not sure about attacks per round) And second, what are the best proficiencies and should I go for dual-wielding or sword and shield? |

I say the Dual-Wield.If you have ToB wait until level 13-15.Dual-Wiel with FoA and Celestial Fury(Did you deserve THE GREAT KATANA???) |

Yeah, I have ToB.
The only problem is that CF is katana, and druids can use only scimitars, clubs and daggers (from one-handed weapons) |

Scimitars and daggers are nice. Try the Staff of the Ram too

Just a quick word of warning, make sure you have sufficient stats to dual to a druid. They have extremely demanding attributes.
You will need minimums of: STR: 15 (minimum primary attribute, preferably 18/xx) DEX & CON not required but should be 18 to tank INT: should be at least 9 so you can use protection scrolls WIS & CHA: 17 (absolute minimums for dual classing to druid) Dual at level 13 to get an additional 1/2 attack per round and approx +2 to your saving throws. Notable Druid weapons: Belm (a must!!) Gnasher +2 (awesome poison damage stacks very fast!) Backblood (excellent troll slayer and 9.5 average damage is not to be sniffed at Personally, I'd dual wield Gnasher (main) with Belm (off). By level 13, you'll have 8 proficiency points: If you have the Grandmastery patch installed, put 5 into clubs, one into scimitar and 2 into "2 weapon style". If you dont have the patch, put 3 into clubs, 3 into schimitars and 2 into "2 weapon style". The GM combo will give you 5 APR with belm, otherwise, you'll get 9/2 APR. With Item upgrade, you can combine the Gnasher and Blackblood into Pitchwife +5

Dual-wielding as Jim describes is probably the most damaging way to go. The only other options with good Druid weapons are Staff and Spear, both two-handed weapons.
Staff: Staff of Rynn +4, Staff of the Woodlands +4 (Druid only, not available till very end of SOA), and Staff of the Ram (TOB only). Spear: Impaler +3 (+10 piercing damage!), not sure about TOB spears. 2-5 points in your two-handed weapon of choice plus 2 points in two-handed weapon style is easily attainable. (You can get most of it when you roll your Berserker!) Your rate of attack will "only" be 5/2 at level 13 Berserker (7/2 with 5 points and Grandmastery patch), but two-handed weapon style doubles your chance of a critical and you get a little reach with your weapons (you can fight from behind your Fire elementals). Don't forget 2 proficienty points in Sling or Dagger if you want a long-range weapon.

I may be wrong about this, but I think Berserkers must be of Chaotic alignment. If this is true, then you will not even be *able* to Dual to Druid, as you will not fulfill the True Neutral alignment requirement.

SixOfSpades: I'm at work so I'm not sure either, but I believe the Berserker restriction is "non-Lawful only", not "Chaotic only". I do know that I have actually created a True Neutral Berserker in SOA with the appropriate attributes. I started a game with him and validated that he could successfully dual to Druid. I haven't done much else with that game; I'm playing another party and saving him for when I get TOB.
