Shoot it! Shoot it LOTS! Works for me! If it gets too close, back off! As far as the problem with psionic blasts (that irritating Hold spell of theirs) goes, any mindshield item will prevent that working. A Helmet of Charm Protection is good, and relatively inexpensive. As or any non-item-based solutions, um... I'm not sure there is one.

You miss out on a lot of cool plot and items if you skip the Underdark. I personalkly finished off all three of the Kings (Elder Brain, Kuo-Toa Prince, Elder Orb) for the XP. It's worth it in the long run!

You have to go through the underdark no matter what but i really reccomend heading back and taking the trip with saemon the pirate its a brilliant quest in this underwater city and you get loads of exp and the most important item the cloak of mirroring. You really really should go back and do that quest.

you have to go through the underdark anyway

Personally, I find mindflayers really fun..free XP and all.
I was using a fighter/assassin (though any thief with a decent THAC0 could do this) and I was sneaking around in the shadows. I backstabbed a mindflayer, ran away (I had the Boots of Speed) and turned a corner, hid again, backstabbed again, and so on and so forth.
I finished both lairs with my single character and was thirsting for more

Why?
