You can add sneak attacks to items, but when you get into item creation, you may find people who consider such a massive enhancement to an item as 5d6 sneak attack as an uber item. If you do that and then play multi, don't be surprised if you are designated by some as a pk'r and are treated as such.

yup, or sneak attack in an item as I mentioned.. however IF your also interested in a high level character and you still want a level or more of Rogue - then put off the rogue class untill much latter and save up skill points. Then place points in the VERY usefull "use any item" skill. I think 12 points in this skill will allow you to use ANY magic scroll.
If you're going to have (more than one) levels of rogue, you might wanna start as a rogue, as you get 4 times as many skill points as usual at first level, and rogues gets lots'a skill points at each level up, then take more rogue-levels later when you can boost use magic device some more.

First thing, assassins don't get sneak attacks as rogues, only the death attacks, which are cool when they work, but...Monk assassins are very potent, if taken with shadowdancer, every attack can be the first attack, if played right. Another possibility is this, which works well in melee, stunning fist, hide in plain sight, attack from flank, death attack against helpless opponent. Works extremely well.

"if taken with shadowdancer, every attack can be the first attack"

not from the description or from what I've experienced.

The key description is: Once the opponent is in COMBAT MODE no more death attacks.

sure, if there disabled on the first hit with stunning fist then you'll get the chance for more attacks.

If however they are not somehow incapacitated on that first hit then they immeadiatly go into combat mode.

Even if you "hide in plain site" they are usually in combat mode (though perhaps just standing still) and you'll get no death attacks.
