Welcome to IronWorks, SrSmacks!

How did your character die?

Certain forms of magic, Death Spell or Finger of Death for example, do not simply kill a character but utterly destroy him so that resurrection is not possible. In regular AD&D a wish spell is typically required to restore such a character to life -- that is not a spell that is available in IWD. If your character was killed by death magic such as the above spells the only way of getting him back is by reloading the game to a point prior to his death.

According to one of the walkthrough I consulted, the idol in that area does in fact cast various death spells. My guess is that this is what killed your character. As I noted above, certain death magic kills your character without any chance of resurrection afterward -- that would be why the portrait is removed.

SrSmacks:

I have had this happen. Recently it has happened in HOW due to some unruly Armor i found. Have you played HOW and used that very Armor?
Another thing: When a character is chunked, he doesnt disappear from the party portraits. Usually i have seen a char reach -12/97 HP when blown apart. Then Ressurection works. But if he disappeared from the portraits, yer outta luck. Reload, or just attack the idol (and interupt his death giving spells). Lata
