First of all, you must free D'Soto in Shurugeon Castle. Speak with him, tell him "Mavin" and then select from your answer list "Erathsmedor", "Elseramavin", and "Shroud". D'Soto will tell you "Go now and claim the Mavin..."
Now, take the Signet Ring and the Shroud of Elsera and go to Erathsmedor. Tell Erathsmedor "Elseramavin". Erathsmedor should make "Eh?!" and want the shroud. Give Erathsmedor the shroud. Erathsmedor opens the way to a room with the sword. If Erathsmedor does not make "Eh?!" then something is wrong. Most likely, you haven't spoken with D'Soto about the essential words. Or you haven't finished the ritual with D'Soto, i.e., you haven't heard "Go now..."

Welcome to the boards, MJG! May life in the Gael Serran bring you fame, fortune, and fun...
Radek's given you the good path. Note the important part -- you don't have to take the sword off of Erathsmedor's body. You just have to give him the right thing and say the right word, and away you'll go! BTW, you may want to create a portal after you leave Erathsmedor's mountain lodge. Once you fall, it's a pain to get back up

Perhaps giving the Shroud of Elsera is the thing that gives you probelms? It seems to be the middle part of an NPC's body that is the area one has to be close to in order for game to consider the character "close enough" to the NPC. Since Erathmedors neck is somewhat protruding you have to stand almost inside the dragon to be close enough for him to accept the Shroud.
