Thanks for the info, I realized that my Rogue who is a Ninja could also do the same but by the time I got to the end of the game she did not have that level of Moon Spells to do the portals. My wizard and bard did though.

It's quite simple to cast this spell : you just have to choose a target to cast it : either the spell caster or a member of your party. In fact, you can't cast the spell if you don't choose a target.

As a gaming practice, I have the spellcaster cast it on themself. That way, I don't have to worry about overwriting a previous portal (as much).
Nothing worse than inadvertantly casting Create Portal at three different key places, only to discover they'd all been cast on the same character

Can you actually have more than 1 portal? I've always assumed that all charactors teleported to the same portal. That could be what happened when I thought that teleport didn't work in Collasium, I was just using the wrong charactor to cast teleport.

The rule is one portal per character per scene. What's a scene? Outdoors is one scene, each dungeon is another... basically, when you have to wait while the game loads the environment for your next steps, you're entering a new scene. You can only teleport to the portal you created in this scene (whatever one you're currently in).
You can look through the best tips thread for more details on portals and teleporting. Lots of good stuff there... Basically, cast create portal on yourself, and when it's time, cast teleport. Most of the time, that works. Be warned: there are some times (and I don't know what causes them) when all your portals will get wiped, and you'll have nothing to go back to. If you're lucky, it will happen when you're out in the wilderness and you can reset at least one portal. If you're not, it happens on the dock in Brimloch Roon and you have a long journey ahead of you...
