As far as I can remember, you need to follow the instructions in Yakman's journal until you reach the first demons, who have the first gem, then go directly "south" until you find the next group, these have the second gem, I usually just kill both groups, now, run into random portals until you get back to the beginning, now follow the instructions until you reach the Cambion and his group, get the third gem from them, do it either by gambling or by killing, you should now have all three gems.
