I lost the pommel jewel of the Equalizer in this way:

In the cutscene where Irenicus steals my soul (right before going into the Underdark all my inventory slots were full.

Then Yoshimo dies and I (automatically get some of his items forcing me to drop some of the existing ones (including the gem which are then irrevocably lost.

That happened to me too. Items were dropped inside that glass jar, and were there still, when I came back to fight Irenicus. I saw them but I couldn't get them. Stupid.
