*Yoshimo (An issue of contention for two reasons.You can bug exploit to keep him, and if you never go to the coronet after leaving him in Athklata, you won't see him die. However, it is made clear that his death is a certain result of the geas spell he is under and his death is part of the storyline, therefore I am including him as an answer to this question).

Common wrong answers
2. Assassin in Brynlaw (who can be avoided altogether), Irenicus's Black Dragon (which can be persuaded)

3. Anyone who dies in a dream sequence on resting (you could get through the game without resting), Phaere and Ardeluce (you can get out of the underdark without doing the scrambled eggs quest), Dill (who doesn't actually die with the other spellhold inmates)

You need 18 charisma.

I've played with 18 charisma and have never been able to convince him. Do i need to be evil or something?

Doh! Missed my chance. Oh well, at least I can point out that you must also kill all 4 Mephit Portals, and that Bodhi can also fall in the "killed by someone other than the party," given the potential for assistance from Drizzt / Paladins / Shadow Thieves.
