How do I Make a Dual Class Character

I just got SoA and I want to eventually make a Kensai into a Mage. So here are a few questions.
1. Is it possible?
2. What do my abilities need to be set at?
3. What level should I switch?
4. What are the the prime requisits I hear about?

1) Yes
2) 15 Strength and 17 Inteligence (i think)
3) sorry, never played this class, but i think maybe 13
4) no clue

For 3) I think it can be 9 or 12.

If you have ToB, level at 13 for the extra attack.

Without ToB, level at 9.

for 2)

As for all dualing chars:

15 in the prime requisits of your old class and 17 in the prime requistes of your new class.
