So, How is it done? Is there a special Icon or something...or do you have to use stealth to get behind the enemy? I have not been able to get this to work with thief class.

Its an option that's enabled in the IWD configuration utility. Its on the same page as the "rest until fully healed", and "max hit points on levelling" options. It ALSO is only accessible with HOW installed.
