there are some perfect guides out on the web that answer that question
take a look at Gambanshee's section "Cromwells Forge" for example. But since I'm borde I'm going to line them out for you (hope I remember everything correctly) item SPOILERS ahead - - - - - - - - - - - - - Gesen Bow - made from Gesen Shaft (Tanner's shop) and Gesen String (Spellhold Maze) Mace of Disruption +2 - Made from MoD+1 (Bodhi's lair) and real Ilithium (Sir Sarles temple quest) Wave+4 - Made from Wave Shaft and Wave Blade (Astral Prison and Sahuagin City) Silver/Vorpal Sword - made from Blade (given to you by Saemon Havarian) and Hilt (on Githyanki that attack you once you are in posession of the blade) Equalizer - Pommel gem (Irenicus maze), Hilt (Mind Flayer Dungeon in the underdark), Blade (Elder Orb in beholder lair in the underdark) Ankheg plate - ankheg scales (from any ankheg in Windspear hills) Red Dragon Armor - red dragon scales (from Firkraag the red dragon in windspear hills) Shadow Dragon armor - shadow dragon scales (from Shadow Dragon in Temple Ruins near Imnesvale) and finally Crom Faeyr - Girdle of Frost Giant strenght (Demon Knights in Kua Toa lair in the underdark), Hammer of Thunderbolts (Mind Flayer lair in the sewers), Crom Faeyr scroll (Shadow Dragon), Gauntlets of Ogre Power (on Halfling in Planar sphere) Now if you can't make anything out of these directions feel free to ask (how do I get into the mind flayer sewer lair? for example)

Ironically, you listed the one question I was going to ask: How do you get into the mind flayer lair in the sewers? I can see what I think is the secret passage, but it seems forever magically sealed.

Go to the Windspear Hills, do the quest, kill the bad boy. Check his body.

~ ~ ~ ~ ~ ~ ~ ~ ~ ~ Kill Tazok. On his body is the sewer key. Go to the temple sewers and near the top there's a secret passage that the key will open. Go inside and beat up dem mind flayas.
