My first time thru BG2 I got into Spellhold via the wardstone you get from Prebe(?), and went in the front door. This time I got in via the Pirate Lord by claiming madness. By getting into Spellhold with the *madness* trick you seem to miss out on the sea trip where you get sunk and have to deal with the Sauhagin(spelling?) to get to the Underdark. So, do I just miss this part? No big deal if I do, just wondering.

You didn`t miss the trip to Sahuagin City. When you finish Spellhold you will meet Seamon and then follow the conversation. Then depending on what you will say you might miss the city or not. But I strongly suggest you go there.

OTOH, if you take the wardstone route, you miss out the opportunity to talk to the inmates (different conversations from when you're getting the "tour" if you use the wardstone) - some of them have some great comments & you can get some items from them - gems from Naljer & Dradeel's Recipe Book, for example. If you plan on taking Imoen with you, you're better off getting into the Asylum by talking to the Pirate Lord.
This is only in a powergaming sense, however, since you can get gems and notes (as Imrahil noted) from the inmates, and you can also get the XP from killing the Cowled Wizard with Imoen in the party after you've left Spellhold.
