ive got 2 pieces of the medallion i need to pass the door in the crypt but i cant find the third any help ? cheers

There's a statue in a room just beyond where you can find the NPC. That has to do with one of the pieces. I take it you've found the floor puzzle one. I think there's another piece near the lava lake.
