Tear him a new anus is the context I've always heard it in. Now, I think the implication to cause massive pain in the lower GI tract, and as such, to vindicate oneself upon said person.
An idle threat most of the time. Mmm...I didn't know they didn't have that one in Ireland.

I believe it means tearing someone a new arsehole. I've heard "I'll tear him a new arsehole" which I believe means "I will cause him some pain"
