Who is the most annoying NPC ever? I'd have to say the Mission Vao/Zaalbar combo. They can't fight, there dialogue is annoying, and I wished to God that I would be able to kill them whenever they started their dialogues.

Several came into my mind:

"Mud" from Gothic 1 : He is there for a purpose--to annoy players.

"Knight" from Wing Commander 1: Always get himself into trouble and he is such a lousy wingman, always get into my way when I am trying to make a kill.

There are more, but these two are the ones I remember so far...

Well I would say Mud... but like you say, hes there just to annoy you. Hes hilarious actually. Hes be more like " the best annoying npc"

The most annoying NPC's in any game would have to be the stupid piper in Black and White or Vinnie Gognitti from Max Payne 2.

Definitely Lynette, the mayor of Vault City, from Fallout 2. ;)

Yeah, that arrogant b**ch. I once lost my cool and took out my .223 pistol and blew a big chuck out of her. Damn her, had to reload the last savegame from hours ago.
