Timber, I meant using two Knights to cover for each other. My personal preference is to use them on both sides of the board while keeping them close enough to cover other pieces.

Neither is strictly necessary. The way I normally do it is to move the bishop out through the opening on e2 after playing e2-e4. Of course, moving your bishop that way is a valid move, and I know some people who play it regularly... Not something I can give advice on, however. Personally, I only move the f, g, and h pawns for a specific attack or defense I need to make, and then only rarely.

Depends on the situation. It can help if you need to defend them from some attacks or if they're likely to be attacked, or it can just end up as a waste of time. Can't really generalize.
