I just installed CEP and a bunch of new mods and stuff, as well as the 1.64 patch. When I used it then everything looked great. Now when I load up this mod or any other (or even the OC) all my new portraits are gone. WTF?

Because this is a server vault MOD you can only select Portraits that are on your local machine.

There are a number of work arounds for this that involve copying your CEP Portraits into your portraits folder.

To add a little if you go to MOD sites that don't have the CEP loaded on their MOD your CEP Portrait won't show up even if you do have them loaded in your Portrait folder. You'll just get the ugly bald guy as the generic replacement(I hate that portrait).
One more thing when you are playing on the undermountain server and you see other portraits of players in the game and the picture is blank,it's because they are useing portraits they have created themselves that are not in the offical game or the CEP.
