I read something about how this overwrites the descriptions from custom items. What does this mean, exactly? Is it talking about items created from scratch, or items in the game that have been modified (like one I changed to match the item's description)?

It will overwrite ANYTHING that you've changed or created. If you want to keep the old ITM files, then zip them up and then unzip them back into your override.

It's a brilliant program though.

But what does it overwrite them with? And what if I've edited an item that is supposed to be fixed by the program?
