AvP is so scary I've never completed the Marine campaign, I just couldn't stand those alien things suddenly jumping out of vents and through the walls and holes and whatever else.

Max Payne isn't a scary game in itself, but it does have some scary aspects, the chapter two dream in Max Payne, for example was pretty freaky.

Carmageddon 2 kind of freaked me out the first time i saw it played. well, i guess it didn't really scare me, just shocked me in its explicit, gory nature.

and then i started playing it myself, and was stuck there for a good hour or two. fun game! but twisted

Clive Barker's 'Undying' is so incredibly scary. I used to play it at 2am with no lights on and the sound way up.... and to see these monsters leap out at you like that.... my heart was in my mouth all the time while playing it

May sound lame, but for some odd reason, when I played Might and Magic 7, those goblins in the beginning make this sound, and are creepy to look at too. Again, dunno why, but I eventually had to turn the sound down, cause it just creeped me out too much LOL. ack!

System Shock ( the first one ) had more than its fair share of creepy moments.
