Question Mark

Reputation isn't all that important in IWD, so I started wondering "just how evil can your party be?". If you wait and finish all the stuff in Easthaven, you cannot attack any citizens because you can't leave for Kuldahar without speaking to Hrothgar. What a shame.
Perhaps you can kill the citizens that have gone missing from Kuldahar, once you find them. I don't know if anyone in Kuldahar will know--how could they?
Once you speak with Larrel for the first time, I think you can really upset him on subsequent visits...but I never have.

Right now I can't think of anything else to try. Any other suggestions about how to make everyone's day a little worse?

You can kill off the adventurers in Dragon's Eye, and you can probably kill off some or all of the deep gnomes in Lower Dorn's once you've done their quests. Don't forget the crippled gemcutter inside the building and the two gnome sculptors in the garden.

Could also expose or kill the escaped slaves in Wyrm's Tooth. Maybe also the priests in Lower Dorn's once you destroy the Idol.

Once you've done all you need to do in Kuldahar (after finishing Orrick's quest), you could probably wipe out the whole village if you were so inclined.

Mad

You can tell the ogre in the tower in Kul. Valley the cure for his headache, then kill him.

You can threaten the little kid who lost his fish to goblins.

But none of that compares to IWD 2, BG series, or, the most evil of them all, Morrowind...where you can depopulate a whole continent, from the gods on down .
