In Chapter 5, the Underdark, Southern Tunnels (Beholder Hive). Just as you enter, you'll run smack into an Elder Orb (on your left) and a Beholder (on your right). The Elder Orb drops the Amulet. Is that "exact" enough for you?

Oh, and you might like to know that when you enter the Tunnels, your party will be facing the wrong way: Your casters are in front, with your Tanks in the back. You might ("might?" make that "will") want to shuffle your Party Order before you go in.

[ADD:] By the way, the ANSWERS to the GBPitBSoATQ have been posted.

Well now actually, when I arrived in the Southern Tunnels there was a Death Tyrant on the left, and a Beholder on the right. Neither one of them dropped the amulet...

Hm. Either you have the Smarter Beholders component of the Tactics MOD installed (which makes some changes to the Beholder Hive, and might very well have moved the Amulet to somewhere else), or you just plain didn't see the loot. Since you say it's a Death Tyrant, I'd say it's the former.

Thought: What the hell would a Beholder do with an amulet anyway???

Yeah, it's Tactics. Or Smart Beholders, if you wish. Among other things, it moves the Hive Mother into the furthermost "eye" on the map, and places a Death Tyrant instead.
