It has happened to me. On 2 chars.
And I know it has more than 1 floor. I've been to all 3 floors

Universal advice for The Party Poisoning Quest
1. SAVE before entering BG city 2. Do the Marek and Lothander encounter when you FIRST ENTER BG city, to get it out of the way. 3. Resume game after having everyone drink of the antidote. 4. Rest for 15 days to make SURE the quest was solved in the proper manner. 5. Go about the business of finishing the rest of the game. That should do it.
