my favorite? yeeeesh. that's a toughie.
i love the baldur's gate 1 intro because it scared the pants off me when i first got it. oooh! but i think sanitarium is still my favorite. it is just so... well... you know... crazy. most memorable part of any game intro? "war." "war never changes." from fallout 2 special mention: arcanum

The thief games intro's are nice and creepy.

I still love the intro to Jagged Alliance 1. It really gave you a feel for what it was going to be like fighting in the jungle. I remember jumping back in surprise when the guy doing the hunting was then snuck up on and shot. Fallout was pretty good also.
