IIRC They are triggered by how long you're into the story (chapter)

True. Each time you start a new chapter, your next Rest should trigger a new Dream.

I think you get different dreams based on your aligment. I started an evil game, and int he first dream, my character said he wanted to get as much power as saverok and was getting used to seeing Gorion getting killed.

I don'T remember ever getting such a dream with a good character.

The dreams and the specials gained are dependant on alignment. The dreams come within the first few rests, not always the first... unless there is a special way you have to rest (outside, in a inn, etc). I rarely have the dream in the first rest after a chapter.
I've also heard (don't know if it's true or not) that who gets the special ability is dependant on the leader of the party at the end of the chapter. So if Imoen is leading your party at the end of chapter 1, she'll pick up the Cure Light Wounds special. I've yet to try this, so I'm not sure.

If it does happen, it is in unpatched game. Although I don't remember such thing even in my pre-patch games.

Whoever told you that, Halbarad, was wrong. You are the child of Baal and therefore you are the one who gets the special ability. I couldn't really imagine Kagain casting Cure light wounds.

Like I said, I'm not sure if it's true. I would have assumed it would be a bug, as like you said, it'd mess up the story. Actually, I think I remember reading it on here the last time I played BG (about a year ago)... but maybe not. Anyway, my bad. Thanks for clearing that up.
