I'm 98% sure it happened in the game that I'm playing now. I'm reserving that 2% in case it was the last one with my full party. It's listed in Shadowkeeper. It is also in the Vesus Books Perfect Guide. I know I've had them before, but I'm not 100% sure that's where you get them. I'm going by the cheat book.
