When I was there the trap door opened after talking to the shaman. Be sure and ask about all of the key words then turn around and the door should be open in the middle of the hut. It will be a long hike to the toadem, which is outside the village proper at the end of a woodland trail. When you have killed the scabans have each character touch the toadem to receive 5 extra hit points.
