1) 12 eyes
2) they make u reload your game so much they should be called reloaders instead of beholders, i got answer by counting number of eyes in picture inside book.
3) In BG 1 u killed Sarevok and saw his soul get destroyed, yet come across him in BG 2?? How can this happen?
