OK, you have to enter the cavern in the south and also to the east there are the main Dorns keep etrance.
in here you will have to fight alot and after that you have to find a really bugging trick to find the entrance to lower dorns deep.
The hint is that in the room where you get nailed by lightningbolts, this is a gigantic staircase and you have to step on certain "plates" in the ground, see the table in the room before this one..
There you have the exit. I hope I rremember this one correctly...
