Icewind Dale, BG1, and BG2 all use these dimensions:

Large size portrait: 110 x 170
Small size portrait: 38 x 60

all portraits must be in .bmp format

Icewind Dale 2 uses these dimensions:

large size portrait: 210 x 330
small size portrait: 42 x 42

All portraits must be in .bmp format

ALL of these portraits use the same naming convention:

either xxxxxxL.bmp or xxxxxxxS.bmp, where the x's is the name of the portrait and can be anything you want up to 7 characters long. The total file name must include the L or S and must be no more than 8 characters long.

Neverwinter Nights portrait file sets must be in uncompressed .tga format, and contain 5 different files. Ziroc posted a link to a NWN template set some time ago.
