1) Sounds like a video card problem. Make sure you have the latest drivers and try selectively enabling/disabling the display variables under "options". You will also want to download and install the patches.
2) Yep only two chars per single player game, but you can have any number of sp games with any combination you like (if that makes sense
)
3)in a readme file or in .PDF format - just guessing.
