You can start the HoW game anytime by going to Kuldahar.
Or if you finish the game, your characters get automatically exported before the boss fight. You can then start a new HoW-only game and import them.

I suggest doing the Kuldahar route and do it at the last moment possible: after you obtain the 6 badges, but before starting the final areas (there is no going back once you start the final areas).

Also, don't forget the TotL expansion!
