Hmm. As far as I remember, there's a glitch with Lelliana. That if you max her disposition before doing the "Assassin" encounter, I believe, that the romance doesn't work out as planned.

I think there was a fix for it as well. I'll do some searching around in the morning (Very late now, and hopped on to see what was up here), unless somebody else has already answered.
