The urea in cosmetics is not biologically obtained, it is manufactured commercially, which is quite easy, as it is a simple molecule,CO(NH₂)₂ . It is also used in fertilizer and animal rations. On the other hand, the estrogen in Premarin is derived from pregnant mare's urine. (Get it? PREgnant MARe urINe.)
