I lost the pommel jewel of the Equalizer in this way:
In the cutscene where Irenicus steals my soul (right before going into the Underdark all my inventory slots were full.
Then Yoshimo dies and I (automatically get some of his items forcing me to drop some of the existing ones (including the gem which are then irrevocably lost.
