The Chair is in the safe room in the slime area. You need a wardstone (i Think it was the compass wardstone) to open the safe door. And the door to level -5 is opened, if you were the 3rd time in the Golem chamber. If you after this talk to Durlag, he will open the door. The 1st time, I played through Durlags tower, I didn´t talk to Ike in Level 0 and anyway got in Level -5 to kill the Demon Knight.
