Actually the method to obtain the last (fire) piece of the flail is rather illogical.

There is a big room behind a secret door, a kind of chapel or shrine. The room contains a bunch of golems and three statues. The statues contain several items. One of the items is the third head of the Flail of Ages. If you take anything but the flail head the golems will go hostile. Taking the flail head will not provoke any action.

This must be done before completing the quest (going outside after killing the Tor-Gal). Otherwise both the golems and the items will have vanished.
