Neither side will "lead off" with a nuclear strike, in any event. The one who launches first will immediately become the "bad guy" to the rest of the world and lose the political/media war; all their allies will abandon them.
Should the two go to all-out war, they will begin with purely conventional forces, or even chemical if they are sneaky can control the story in the media.

No, there won't be any "domino" effect. Even if both countries push "the button", no other sane country will. No one would want to take the risk of following the other lemmings off that particular cliff.

The only thing I haven't been able to figure out is which area will get hotter faster: Israel/Palestine or Pakistan/India?
