Nope, when there's no heirs, your bloodline dies, and your kingdom and armies will become rebels. In other words....game over.
They already had those rules in Shogun Total War, and hasn't changed in Medieval. Kinda silly if you ask me, in real life, there were dozens of nobles, or even armygenerals lining up to take over the throne.
